It's really just a matter of context, and when the context is Minecraft, a mod is internal only.
Things like resource packs, which just change the textures and sounds of the game, or command block contraptions, which can accomplish a lot while keeping the internal structure the same, are not considered mods. This would be to add new items/mobs/mechanics/dimensions/etc. However, given the open nature of the game, the term 'mod' in Minecraft almost always refers to actually changing the game's code. And yes, 'mod' is just short for 'modification,' which all these things are. It's a sandbox game, so you should be able to change whatever you want about it.
